ABSTRACT

Objective:To explore the application of expanded polytetrafluoroethylene (ePTFE) as the main support combined with a small amount of ear cartilage in rhinoplasty. Methods:Through a nasal opening approach, the ePTFE was used as a support implant for the nasal dorsum and columella, and unilateral concha cartilage was used as a nasal tip modification graft. Results:This method was applied in 56 cases of rhinoplasty (36 cases of initial nose, 20 cases of silicone augmentation rhinoplasty). The wounds of 55 patients healed at one stage without complications, with satisfactory results (average follow-up of 6 months). In one case, nasal mucosa was damaged early after surgery, and the ePTFE was partially exposed, which healed after debridement and suture. Conclusions:For most primary rhinoplasty and simple repair of the nose, using ePTFE as the main support combined with a small amount of ear cartilage, has the advantages of fewer materials, rapid surgery, mild trauma, and stable postoperative results.

ABSTRACT

Objective:To analyze the clinical application effect of otoendoscopic myringoplasty with tragus perichondrium. Methods:The clinical data of 20 patients with tympanic membrane perforation who received treatment in Suixian Traditional Chinese Medicine Hospital from March 2019 to March 2020 were retrospectively analyzed. Among the 20 patients, 6 patients had traumatic tympanic membrane perforation, and 14 patients had tympanic membrane perforation caused by chronic suppurative otitis media (stationary phase). All patients underwent otoendoscopic myringoplasty with tragus perichondrium. After surgery, the healing of tympanic membrane perforation and the improvement of hearing were evaluated. Results:After surgery, all patients were followed up for 12 months. Traumatic tympanic membrane perforation healed in six patients. The tympanic membrane grafts survived in 12 patients with tympanic membrane perforation caused by chronic suppurative otitis media (stationary phase). One patient had poor postoperative healing and the remaining small perforation healed after secondary treatment under local anesthesia. Healing from tympanic membrane perforation was not achieved in one patient because of perforation caused by otitis media. The healing rate of tympanic membrane perforation caused by chronic suppurative otitis media was 95%. Before surgery, the air conduction pure tone hearing threshold was (42 ± 11) dBHL, and it was (25 ± 10) dBHL 12 months after surgery. There was a significant difference in air conduction pure tone hearing threshold between before and after surgery ( t = 4.00, P < 0.05). No complications such as tragus cartilage infection, skin flap necrosis of external auditory meatus, peripheral facial paralysis, hearing loss, or tinnitus occurred in 20 patients with tympanic membrane perforation. Conclusion:Otoendoscopic myringoplasty with tragus perichondrium leads to a high perforation healing rate, improves healing greatly, has no serious complications, produces minimal trauma, and contributes to a rapid recovery from tympanic membrane perforation.

ABSTRACT

Full?thickness deficiency of eyelid tissues can result in coloboma or retraction or both. Here we report our initial experience on the use of auricular skin?cartilage sandwich graft technique for full?thickness eyelid deformities. Five patients (4 years) underwent the procedure. Patients with full?thickness eyelid deformity were included. Three patients were operated for large?sized coloboma and two for eyelid retraction. One patient had congenital, and four patients had acquired etiology. The following parameters were specifically assessed: correction of deformity, ocular surface problems, graft status, and epithelization of skin?cartilage graft. All the patients had a good correction of eyelid position, except one patient who had severe eyelid retraction (8 mm) at presentation. None of our patients had corneal erosion/defect, persistent ocular surface redness, or graft loss. The auricular skin?cartilage sandwich graft technique produces optimal results with no graft loss. Advancement of orbicularis muscle in between the auricular skin and cartilage grafts (sandwich technique) is an imperative step that leads to the survival of both grafts.

ABSTRACT

Objective:To compare the pros and cons of harvesting ear cartilage through anterior and posterior auricular approaches during rhinoplasty. Methods:From January 2017 to December 2018, 63 patients with otochondral rhinoplasty in our hospital were enrolled in this study, 60 were female and 3 were male; the average age was 31.6 years (range, 18 to 43) . They were randomly divided into anterior auricular approach group with 32 cases (64 sides) and posterior auricular approach group with 31 cases (62 sides). Surgical duration, complications and postoperative scar of the two methods were analyzed. Results:The average time for harvesting the ear cartilage through posterior auricular approach and anterior auricular approach was (20.8±1.7) min and (12.6±1.1) min, respectively ( P<0.01). The overall complication rate was 15.6% for posterior auricular approach group and 4.8% for anterior auricular approach group. The wound healed well in both groups, and there was no significant difference in postoperative scar between the two groups during an average 13 months follow-up period. Conclusions:While both the anterior and the posterior auricular approaches present with similar inconspicuous scarring, the use of anterior auricular approach alone to harvest ear cartilage during rhinoplasty provides both the surgeons and the patients with easier access, shorter surgical duration, and fewer complications. Hence, we believe that the anterior auricular approach possesses greater advantages than the posterior auricular approach.

ABSTRACT

Objective:To discuss the principle and effect of augmentation rhinoplasty with auricular cartilage and expanded polytetrafluoroethylene. Methods:From January 2018 to January 2020, 161 patients (10 males and 151 females; aged from 19 to 48 years, with an average of 26 years) underwent " auricular cartilage plus expanded polytetrafluoroethylene" augmentation rhinoplasty in Nanfang Hospital of Southern Medical University. The expended polytetrafluoroethylene was carved into a willow leaf shape (I Shape) to fill the nasal dorsum, and the cartilage taking from cymba concha was constructed into an arched bridge shape for the nasal tip shaping. Pre-operative and 1-year post-operative measurements nasal length, nasal height, nasal depth, nasal columella height, nasal tip width, nasofrontal angle, nasolabial angle, survey of satisfaction and complication rate 1-2 years after operation were taken. The statistical analysis of nasal morphological indicators and nasal aesthetic indicators were employed. Results:The nose shape of 161 patients was improved to varying degree. All morphological indicators were improved, and difference was statistically significant ( P<0.05). The nasofrontal angle reached the standard in 90 cases, accounting for 55.9%; The nasolabial angle reached the standard in 143 cases, accounting for 88.8%. 2 cases had prosthesis (ePTFE) deviation and were corrected by surgical repair; 1 case had prosthesis (ePTFE) rejection and was corrected by prosthesis (ePTFE) removal surgery. Conclusions:Corresponding to the anatomical characteristics of the external nose, the prosthesis material is designed and made to correspond to the dorsum shape of the nasal stent. The shape of the alar cartilage, the prefabricated arched bridge shape of the cymba concha cartilage are used to reconstruct the nasal tip, which can effectively elevate the nasal dorsum, improve the protruding degree and rotation degree of the nasal tip, and have good long-term support. The flexibility and activity of the nasal tip are similar to the biological nose.

ABSTRACT

Objective:To use three-dimensional (3D) scanning to measure the preoperative and postoperative nasal parameters of the patients received rhinoplasty with ear cartilage and silicone prosthesis, and to evaluate the clinical effect of the surgery. Methods:Sixteen female patients with an average age of 28.3 years, ranged from 21 to 35 years, received rhinoplasty with ear cartilage and silicone prosthesis in Wuhan Tongji Hospital from June 2018 to February 2019. Preoperative and postoperative 3D scanning was performed to measure nasal parameters, including linear length, angle, and proportional index. Results:All patients were satisfied with the postoperative outcomes. The postoperative nasal length, nasal height, and nasal depth increased significantly, and the postoperative nasal width and nasal tip width decreased. The postoperative nasolabial angle and nasofrontal angle were statistically improved, while the preoperative and postoperative data of columellar facial angle, nasal tip angle, and nasal column-lobular angle were not significantly different. The ratios of nasal depth and nasal width, nasal index and nasal tip protrusion were improved after surgery, while the postoperative ratios of nasal columella and nasal lobules length were not statistically improved. Conclusions:The 3D scanning allows for comprehensive and accurate measurement of the nasal parameters. The rhinoplasty with ear cartilage and silicone prosthesis is more effective in improving the overall contour of the nose, but less effective in improving the aesthetics of the nasal tip.

ABSTRACT

Relapsing polychondritis is a systemic, chronic and autoimmune disease characterized by the inflammation of cartilaginous tissues. This disease is associated in a few cases with malignant hematological diseases. We present a case of a patient with relapsing polychondritis and concomitantly with acute myeloid leukemia. A 26-year-old female patient, with cutaneous affection as initial manifestation categorized as erythema nodosum. Then she was diagnosed with acute myeloid leukemia. In the aplasia post-chemotherapy phase, the patient developed bilateral inflammation of the ear cartilage (auricular chondritis) and a vertiginous syndrome with satisfactory clinical evolution to immunosuppressive treatment with glucocorticoids. Conclusion: Relapsing polychondritis usually presents with cartilaginous involvement, such as bilateral atrial chondritis, as shown in the case. Early diagnosis and timely treatment are necessary to achieve a good clinical response. Subsequent studies are necessary to evaluate the association between relapsing polychondritis and hematological alterations such as acute myeloid leukemia and the use of chemotherapy(AU)

ABSTRACT

The reconstruction of the secondary lower eyelid to a resection of skin cancer is a challenging surgical procedure that must be carefully planned with regards to issues related to lamella and extension of the resection. We present the case of a 69-year-old male patient who, after oncologic resection, presented a total thickness defect greater than 60% in the lower eyelid. For reconstruction, it was indicated that the use of Tripier's unipedic myocutaneous flap with scapha cartilage graft produced favorable results, which confirms that it is worthwhile using these techniques when planning lower eyelid reconstruction.

ABSTRACT

Cryptotia is a congenital ear deformity common in Easterners and rare in Westerners, with most studies addressing Eastern surgical techniques applied to children. In this pathology, the cartilage of the upper pole of the ear is lodged subcutaneously in the temporal region, which prevents individuals from using glasses due to lack of support and prevents esthetic definition of the upper pole. The present study aimed to report the case of an adult patient with cryptotia undergoing surgical treatment using a mastoid subcutaneous pedicle flap and review the main techniques described for the treatment of this involvement. The subcutaneous pedicle flap described by Yoshimura proved to be adequate for correcting cryptotia in a Western adult patient.

ABSTRACT

ABSTRACT Introduction: The aims of auricular reconstruction are to maintain the permeability of the external auditory canal and restore general shape and projection. Objective: Characterize a clinical case of posttraumatic reconstruction of a partial ear loss, highlighting the importance of the therapy sequence followed. Clinical case presentation: A male 26-year-old patient attends outpatient consultation. He reports that he was treated in the emergency service 10 days before upon having lost a section of his ear due to a bite in a fight. Physical examination reveals irregular partial loss of the right ear. It is decided to wait three weeks and indicate preoperative checkup. At the end of that period the first surgical reconstruction session is conducted, with autologous graft of auricular cartilage from the left ear, which was placed in the form of a "pocket" in the temporal region. Three weeks after the first surgical session, the second reconstruction is performed under local anesthesia, with release of the "pocket" from the auricular region, advancement of a posterior flap and placement of a free skin graft. The patient reported great satisfaction with the esthetic results obtained. Main remarks: A therapy sequence was followed in response to a partial traumatic defect of the outer ear in its upper and middle thirds. This type of traumatic sequel required the performance of surgical procedures at several operative moments, ensuring good blood supply to tissues, absence of tension and appropriate final esthetic results(AU)

ABSTRACT

Objective@#To investigate the histological and biomechanical changes of rabbit ear cartilage induced by long pulse width 1064 nm Nd: YAGlaser.@*Methods@#Seven New Zealand rabbits weighing 2.0-2.5 kg, male, 4-6 months old were selected. For a self-control, the right ear was irradiated by laser and the left ear was the normal control group. Each rabbit ear was divided into 3 cm × 1 cm, three 2 cm × 1 cm and 1 cm× 1 cm experimental area. After the experimental area of the laser irradiation group was irradiated by long pulse width 1064 nm Nd: YAG laser, Three pieces of 2 cm × 1 cm cartilage in each ear of the experimental rabbits were immediately cut and stained with HE, Sirius red and Masson to observe the histological changes of chondrocytes. The 3 cm × 1 cm and 1 cm× 1 cm cartilage of each rabbit ear was cut for biomechanical examination, and the changes of biomechanical properties such as tension, fracture, elastic modulus and compression of rabbit ear cartilage were observed immediately after the operation.@*Results@#Histological observation showed that the chondrocytes became small, the matrix was stained deeply, the refraction was obvious/evident and the collagen was relatively increased. After being irradiated by long pulse width 1064 nm Nd: YAG laser, the peak tensile stress, tensile elastic modulus, tensile fracture load, compressive stress peak, compressive elastic modulus and compressive failure load of rabbit ear cartilage in laser irradiation group were (5.22 ±0.80) MPa, (42.40 ±9.78) MPa, (22.86 ±4.85) N, (16.04 ±5.57) MPa, (28.71 ±13.97) MPa, (1 211.63 ±427.86) N. All of them were smaller than those of the control group[(6.07±0.64) MPa, (48.44±6.30) MPa, (26.94±4.19) N, (25.12±9.10) MPa, (45.30±19.24) MPa, (1 962.83±896.71) N], and the difference was statistically significant (P<0.05).@*Conclusions@#After the operation of long pulse width 1064 nm Nd: YAG laser, the stress of rabbit ear cartilage resisting elastic deformation is reduced, the shape is easier to change, and the remodeling effect is more stable.

ABSTRACT

Objective@#To investigate the effects of the repair and restitution of ear-shaped cartilage by adipose tissue-derived stem cells(ADSC) and cartilage acellular extracellular matrix.@*Methods@#ADSC were extracted by digesting with collagenase type II from the adipose tissue from 32 patients with adiposity whose fats were drawn, and were cultured and subcultured in vitro. The natural biological scaffolds were prepared by acellular method using porcine ear cartilage, and then the second generation ADSC(5.0×107/ml) were inoculated on the preformed natural bio-scaffold scaffold by culturing in vitro for 3 days to form a cell scaffold complex. 32 New Zealand white rabbits were randomly divided into the experimental groups, the control group A, the control group B and the control group C. All New Zealand white rabbits were modeled by ear cartilage defects. The cell scaffolds composite was implanted into the experimental group of the ear cartilage defects of rabbits, the ADSC were implanted into the control group A, the cartilage acellular extracellular matrix scaffold was implanted into the control group B and the control group C was modeled only by ear cartilage defects. After 16 weeks, the animals were sacrificed and the repair effect was observed by gross appearance and histological examinations including HE, Toluidine blue staining, Safranin O and typeⅡ collagen staining. Its were quantitatively analyzed by positive staining results of type Ⅱ collagen. Ear cartilage tissue elasticity was detected. SPSS 17.0 software was used to analyze the data.@*Results@#The cartilage defects in the experimental group were repaired well by general shape observation and those in the control group was filled in with granulation tissue. There were significant differences between the experimental group and the control group in the wet weight(P<0.05). HE staining showed that cartilage cavities formed in articular cartilage defects, and only the fibrous tissue was filled with the ear cartilage defect in the control groups. In the repair area, Toluidine blue staining, Safranin O and type Ⅱ collagen staining were positive in the experimental group, and negative in the control groups. There was no significant difference between the experimental group and the normal ear cartilage in the ear cartilage elastic constant detection(P>0.05).@*Conclusions@#The mechanics and histology of rabbit ear neonatal cartilage constructed by ADSC combined with cartilage acellular matrix are close to normal ear cartilage. Cartilage acellular matrix material combined with adipose-derived stem cells has good repair and reconstruction ability for ear cartilage defects, which possesses potential clinical application value.

ABSTRACT

BACKGROUND: Cryptotia is a congenital anomaly in which the upper part of the retroauricular sulcus is absent and buried underneath the temporal skin. Various surgical techniques have been reported for the correction of cryptotia following Kubo’s V-Y plasty in 1933. Conventional methods using a local skin flap, skin grafting, tissue expansion, Z-plasty, and any of these combined approaches can result in skin deficiency of the upper auricle. The aim of this study was to develop a new method that improves cosmetic results and has fewer complications. METHODS: This study involved four patients in whom five cryptotia deformities were corrected using V-Y plasty and Z-plasty. After elevation of the flap, acellular dermal matrix (ADM; MegaDerm) that was over 5 mm in thickness was applied to the cephalo-auricular angle and positioned to enhance the projection of the ear. Lastly, the flap was transposed to complete the repair. RESULTS: Between January 2014 and February 2018, cryptotia correction with ADM graft was performed in four patients. None of the patients developed a recurrence of cryptotia, and there were no postoperative complications such as wound infection, seroma formation, and dehiscence. In addition, the procedures resulted in a favorable cosmetic appearance. CONCLUSION: Based on these findings, i.e., no recurrence and a favorable cosmetic result, when using an ADM graft, it is suggested that this technique could be an alternative method of cryptotia correction. It could also lessen donor-site morbidity when compared to autologous cartilage grafting and be more cost-effective than using cartilage from a cadaver.

ABSTRACT

Objective@#To summarize the clinical effect of using the composite flap of microtic cartilage and soft tissue in tragus reconstruction to correct preauricular depression.@*Methods@#From January 2015 to November 2016, we corrected preauricular depression of 523 patients with congenital microtia that were treated with tragus reconstruction by using the composite flap of residual ear cartilage and soft tissue.@*Results@#All the composite flap of residual ear cartilage and soft tissue were transplanted successfully. No blood circulation disorder and significant contracture occurred. All patients were followed up for 6-14 months with an average of 10 months. The reconstructed tragus looked projective and the preauricular depression was corrected, demonstrating a similar looking to the healthy side. The patients were satisfied with the results.@*Conclusions@#The residual ear tissue could form the composite flap of residual ear cartilage and tissue. Using the composite flap to fill the preauricular depression was effective, which was not easily absorbed. The residual ear tissue is the best resource for filling the preauricular depression. It should not be discarded in ear reconstructive surgery, but reserved for the next operation for tragus reconstruction.

Objective To observe and discuss the application and clinical effect of different forms of ear cartilage in nasal tip plasty. Methods Auricular cartilage was used to prune it as "halberd" shape at first, and then the extended transplantation and the cartilage fornix reconstruction were carried out by using single or combined septum cartilage.Meanwhile, nasal tip plasty was performed by using "cap shape" or "shield shape" cartilage grafting. Results 136 patients were followed up for 6 to 24 months after operation.119 cases were satisfied, accounting for 87.5%,of which 11 cases (ear cartilage weakness) were not improved obviously, accounting for 8%.However,there were only 6 cases with the unsatisfactory feedback as the nasal tip that was decreased by 1~2 mm one year after the operation, accounting for 4.41%.Conclusions The auricular cartilage is pruned as "halberd" shape singly or combined with septal cartilage as an extended graft, reconstructing the cartilage fornix in the meantime, which is the way better than the traditional way of making nasal tip plasty to obtain the ideal nasal tip protrusion.

ABSTRACT

Objective To explore the clinical effect of double M-shaped cartilage used in nasal tip cosmetic plasty. Methods From December 2016 to May 2017,nasal fip plasty was performed in 39 patients in Hangzhou Huashan Luyeme Medical Cosmetology Hospital. With the Goodman's incision in the columella and nasal vestibule,a big piece of autologous nasal septal cartilage was removed and transplanted to the front of nasal septum.Ear cartilage was trimmed into two strips and sutured with nasal septum cartilage graft.Meanwhile, the sandwich cartilage graft was inserted between the lower lateral cartilages to form the first "M" shape stent when fixing part of it on the nasal septum. Then the ear cartilage was turned over and fixed with lateral crus of the lower lateral cartilages to form the second "M" shape stent.Willow-shaped prosthesis was placed into the nasal dorsum. Results A total of 39 cases of nasal tip surgery were performed, including 1 male and 38 females.Ages ranged from 21 to 48 years in this group. The follow-up period was from 1 month to 5 months.36 patients were satisfied with the efficacy while 2 patients were suffered from the deviation of nasal tip,one patient was suffered from nasal septum deviation. Conclusions The application of double M-shaped cartilage in nasal tip cosmetic plasty is one of ideal rhinoplasty approaches with less wound, simple procedure and good clinical effects.

ABSTRACT

Objective To evaluate the biomechanical effectiveness of hyaluronidase (HAase) on rabbit auricular cartilage in the early stage. Methods HAase in 3 different concentrations (75 U/ml,150 U/ml,300 U/ml) were injected subcutaneously to rabbit auricle in 3 groups, while normal saline were injected as control. For the comparison of biomechanism properties among different groups and different time, cartilages were harvested at 3rd and 7th day after injection, followed by stress-relaxation assay. Results Auricular cartilage displayed different levels in control group compared with other groups in elastic modulus (P<0.05) and maxmum stress (P<0.05) in 3th day as well as 7th day. While both in the same concentration group, there were also differences between the 3th and 7th day (P<0.05).Conclusions HAase injection can cause changes in biomechanical properties of auricular cartilage. And 7 days would not be enough for the tissue recovery biomechanically.
